If your cosmetics facility is outside the United States, FDA facility registration under MoCRA requires a U.S. agent — and this page walks through signing up for one, exactly as it happens on this site. The whole sign-up takes about ten minutes: one intake form, a review screen, and a secure checkout. No account to create first, no sales call, and the price is on the table before you start.

Before you start: what to have ready
The form asks about your facility, not your products’ formulas. Having these ready makes it a single pass:
- Facility legal name and address — the manufacturing or processing site being registered, exactly as it should appear with FDA.
- A contact email and phone for the facility (phone with country code).
- Your FEI or DUNS number, if you have one. If you don’t, leave those fields blank — obtaining the FEI is part of what we handle after sign-up.
- Your brand list — up to ten brands, with the responsible person’s name and the product categories that apply to each (the form has checkboxes for the standard categories).
- An authorized representative — the person at your facility who can speak for it, with their title, email, and phone.
- Optional documents — a business registration certificate or product list. You can upload them in the form or send them later; they do not block sign-up.
One scope note, stated on the form itself: this service covers your FDA facility registration and your U.S. agent. Product listings (Form FDA 5067) are not included in this service.
Step 1: Open the intake form
From the US MoCRA Agent homepage, click Get started. That takes you to the customer intake form — the same form this walkthrough follows, section by section. It has six sections, A through F.
Step 2: Section A — facility identity
Section A is your facility’s identity: legal name, any DBA or trade names, street address, city, state or province, postal code, and country, plus the facility’s email and phone.
At the bottom of the section are the FEI and DUNS fields. If you already have an FEI number, enter it — it speeds things up. If you don’t have one, leave them blank. A missing FEI is the most common reason registrations stall, and requesting one for you is part of the onboarding.
Step 3: Section B — your brands and categories
Section B is where you list each brand manufactured at the facility — up to ten. For each brand, add the responsible person’s name and tick every product category that applies. The category list covers the standard cosmetic categories, from skin care and makeup to hair products, fragrances, and baby products.
Accuracy matters here more than completeness tricks: list the brands this facility actually makes, under the names that appear on the labels.
Step 4: Section C — your U.S. agent (already filled in)
Section C shows your U.S. agent’s details — that’s us, US MoCRA Agent — so there is nothing for you to fill in. This is the agent that will be named in your FDA facility registration: a U.S.-based contact for FDA communications about your facility.
Step 5: Section D — authorized representative
In Section D, name the person authorized to represent your facility, with their title, their relationship to the facility, and their email and phone. This is the person we coordinate with on your registration, and the person who can confirm details or approve corrections if FDA asks questions.
Step 6: Section E — documents (optional)
Section E lets you upload supporting documents, such as your business registration or a product list. It is optional at sign-up — you can send documents later — but sending them now shortens the back-and-forth before filing.
Step 7: Section F — attestations and signature
Section F is the attestation block. Read each statement and tick the boxes that apply, then type your full name as your electronic signature and add the date. These attestations confirm, among other things, that the information is accurate and that you authorize us to act as your U.S. agent and prepare your registration. When the boxes are ticked and the signature is in, click Submit Intake Form.
Step 8: Review and submit
Before anything is sent, a review screen shows everything you entered. Check it once, carefully — facility name spelling and the facility address are the two fields most worth a second look. If anything looks off, go back and fix it. Otherwise, confirm and submit.
Step 9: Confirmation and payment
After you submit, you land on a confirmation page — your intake is in. That page has a Pay $450 Onboarding button, and the same payment link also arrives by email, so you can pay now or from the email later.
The button opens a secure Stripe checkout. Enter your contact and card details and review the totals before paying:
- $450 onboarding fee — due today. This covers the intake review, the FEI check or request, and preparing your facility registration.
- $34.99 per month — starts after a 30-day free trial. This is the ongoing U.S. agent service: FDA correspondence handling, registration maintenance, and renewal tracking.
No other charges are added at checkout, and the monthly plan is billed through the same card.
What happens after you sign up
Once your payment clears, onboarding runs in this order:
- FEI check or request. We confirm whether your facility already has an FEI number. If not, we request one from FDA — this is the step with the longest lead time, typically 7 to 10 business days.
- Registration preparation. With the FEI in place, we prepare your facility registration for FDA’s Cosmetics Direct portal from the information in your intake, and confirm anything unclear with your authorized representative before filing.
- Agent of record. US MoCRA Agent is named as your U.S. agent in the registration. FDA communications about your facility come to us and are relayed to you.
- Renewal tracking. Facility registration renews every two years. We track the date and start the renewal with you ahead of the deadline.
Common questions
Do I need an FEI number before signing up? No. Leave the FEI field blank if you don’t have one — checking for an existing FEI, or requesting one, is part of onboarding.
How long does sign-up itself take? About ten minutes for the form and checkout. The overall registration timeline depends mainly on the FEI step; see how long MoCRA registration takes for the full picture.
Can I submit the form now and pay later? Yes. The confirmation email contains the same payment link as the thank-you page. Your onboarding starts once payment clears.
Are product listings included? No. This service is facility registration plus U.S. agent service. Product listings are a separate MoCRA obligation handled by the responsible person; see our FDA cosmetic product listing guide for how that side works.
Is US MoCRA Agent part of the FDA? No. We are a private service, not affiliated with the FDA. We prepare and support your registration and serve as your U.S. agent; FDA systems and decisions remain FDA’s.
Ready when you are
The form is the same one walked through above: start your intake. If you would rather check your obligations first, the free MoCRA readiness check takes two minutes and tells you whether your facility needs to register at all.
